We would like to hear from people who have needed to visit Accident and Emergency (A&E) several times over the past year.
We know that people attend A&E for many different reasons. By understanding your experience, we can improve services and make sure people are better supported to access the right care, closer to home.
We have created a short survey to help us do this. It takes around five minutes to complete, depending on how much you would like to share.
