What is Active Health?

The purpose of the scheme is to improve the health and wellbeing of individuals who have a physical or mental health condition that will benefit from exercise and require support from a structured intervention to encourage a long-term lifestyle change.

Who is Eligible?

Patients can be referred by a Health professional or Social Prescriber, and must be:

  • Aged 14 years and over
  • Committed to making a long-term lifestyle change 
  • Clients must be clinically stable and compliant with their medication
  • Have an existing health condition, recovering from an operation or other risk factors for disease (as set out below)

  • High Blood Pressure
  • Arthritis
  • Osteoarthritis
  • Osteoporosis
  • Asthma
  • COPD
  • Long Covid
  • Diabetes
  • Mental Health
  • Obesity

Healthy Lifestyle changes

Your dedicated instructor will promote healthy lifestyle changes, advise you on how to exercise safely and support you through the programme with any advice you need. How to access Active Health?

To get started please speak to your Health Professional or Social Prescriber about being referred onto our Active Health scheme where you will be able to use a range of facilities with expert guidance and support.
