There is still time to share your views on two important NHS surveys, both closing on Friday 30 January 2026.
Your insight and lived experience play a vital role in shaping local health and care services. By taking part, you can help ensure services reflect what matters most to you, your family and your community.
NHS 40+ health check survey
We are inviting people who have accessed, or have experience of, NHS Health Checks for those aged 40 and over to share their views.
Your feedback will help us improve how NHS Health Checks are designed, delivered and communicated locally.
An NHS Health Check can help identify early signs of conditions such as:
- heart disease
- diabetes
- kidney disease
- stroke
It also offers personalised advice to support healthier living. For people aged over 65, the check includes information on early symptoms of dementia.
The survey takes up to 10 minutes to complete.
You may be eligible for a free NHS Health Check if you:
- are aged 40–74
- live in England
- do not already have conditions such as heart disease, diabetes or high blood pressure
If you believe you are eligible but have not yet been invited, you can contact your GP surgery or the Essex Wellbeing Service on 0300 303 9988 to book.
Survey for people who use multiple health and care services
We would also like to hear from residents and carers who use more than one health or care service.
This survey focuses on the lived experience of people who access services across two or more of the following:
- health services
- social care services
- community and voluntary sector support
Your feedback will help us understand what is working well and where improvements could make a real difference.
